Forms to complete prior to Thesis I term:

Plan of Attack submission to Faculty Advisor:

These files provide the student with templates for the Plan of Attack and associated Gantt Chart. The Plan of Attack must describe in your own words the phases and tasks to be completed during the thesis project. The description in the Plan of Attack must map to the tasks and milestones shown in the Gantt Chart. The Plan of Attack example is written in Microsoft Word. The Gantt Chart tool is in Microsoft Excel.

This form is designated for a title change, faculty advisor change & employer advisor change. Submit this form directly to Thesis Services.

Academic Thesis Initiation Form
Must have approval from Thesis Services prior to submitting this form. All required signatures must be obtained before PTA & Graduation Plan is processed.
