Note that students who fail to provide proof of health insurance each year by their designated deadline date (which will be communicated prior to new students' enrollment) will automatically be enrolled in Maksin's Student Insurance Program and billed accordingly.

Kettering's health and counseling services are free to all students. However, if a student is in need of a referral for a medical specialist, lab work, x-rays, specialized medical procedures or an emergency outside of our center, either the student and/or the student's insurance company will be responsible for the cost.

Please complete the Health and Medical History Form and return it to the university as indicated. Immunizations must be up to date. A TB test is required.
